Abstract: 

باید توجه داشت که دریا درمانی تنها به شنا کردن محدود نمی شود بلکه به چیزهایی که در سطح زیر و پیرامون دریا بوده گسترش و تعمیم پیدا می کند. عناوین و ترمینولوژیهایی که ذیلا به آنها اشاره می شود موید وسعت کاربرد دریا درمانی می باشد.درمان به وسیله رسوبات دریا(slime THERAPY) درمان به وسیله استحمام در دریا (Balneo THERAPY)درمان از طریق شن داغ کنار دریا(Psammo THERAPY) آفتاب درمانی(Helio THERAPY) درمان به وسیله نسیم دریایی(Anemo THERAPY) مهمترین مزیت دریا درمانی برای بیماری های روماتیسمی بخش هیدروتراپی و آب درمانی آن می باشد زیرا آب دریا با داشتن خاصیت شناوری می تواند انسان را در خود معلق نگه دارد (Buoyancy of water) و بدین ترتیب استرس به مفاصل وارد نمی شود، انقباض عضلات را تسهیل و باعث تقویت آنها می گردد واز مفاصل محافظت می نماید. ممکن است بپرسید آیا تفاوت هیدروتراپی و دریا درمانی در چیست؟ اولا دانسیته آب دریا بیشتر است و ضمنا انسان از تجلیلات ساحل لذت برده قدم زدن در آب ساحل و امواج دریا به او آرامش می بخشد زیرا آزادی عمل برای عضلات فراهم می آورد که مجموعا این ورزش مفرح در بهبودی، توانایی و well being بیمار نقش موثری ایفا می نماید با توجه به اینکه سرما و رطوبت دریا درد مفاصل و خشکی عضلات را افزایش می دهد، لذا لازم است رطوبت، درجه حرارت آب دریا، سرعت و جهت باد و فشار هوا قویا قبل از درمان مد نظر قرار گیرد بنابراین جز در مناطق حاره در دیگر نقاط جهان فقط در فصول معینی این درمان امکان پذیر می باشد. اروپائیان برای اینکه دریا درمانی را به تمام اوقات سال تعمیم دهند آب دریا را گرفته، به استخرهای مخصوصی منتقل می کنند و با ایجاد درجه حرارت آب به 37 و برای برخی بیماران به 42-40 آب درمانی را امکان پذیر می سازند.از دیگر خواص شایان ذکر آب درمانی وجود جلبک دریایی بوده که ازدیاد جریان خون در پوست را باعث می شود و روغن ماهی که عمدتاFAW_3  می باشند PGE3 و LPB5 را افزایش داده و در نتیجه بیماریهای روماتیسمی و التهابی را کاهش می دهد.

Abstract: 

Introduction: Musculoskeletal problems are the common complaints of patients refer to internal medicine clinics and the pain is the most important of them. There are different physioTHERAPY methods for reduction of pain and action potential simulation THERAPY (APS THERAPY) is one of newest methods. The aim of this study was to determine reduction of pain with APS THERAPY. Materials & Methods: In 47 patients with different musculoskeletal pain APS THERAPY performed in 6 days each for 16 minutes and with current of 0.7-1.2 mA. Pain (VAS), Global functional status (ACR) and relief of pain (VAS) before and after study compared.Results: APS THERAPY reduced pain (p<0.001) and increased Global functional status (p<0.001) and increased relief of pain (p<0.005) but there was not any correlation between this reduction of pain with APS THERAPY and educational status and past history of physioTHERAPY and duration of illness in these patients.Conclusion: APS THERAPY is a useful physioTHERAPY modality for reduction of pain in musculoskeletal problems.

Abstract: 

Schmidt syndrome is a polyendocrinopathy characterized by multiple organ failures. Patients with two or more of the following characteristics are diagnosed with this syndrome: Graves' disease, adrenal insufficiency, autoimmune thyroiditis, and type I diabetes. The present case report concerns a Schmidt syndrome patient (polyglandular autoimmune syndrome type II). The patient is a 42-year-old woman with type I diabetes and complaints of imbalance, weakness and lethargy, weight loss, darkened skin, blood pressure drop, hypoglycemia, and hyperkalemia who visited the Hospital frequently and was discharged every time after intravenous potassium chloride and dextrose injections and orders to reduce insulin consumption at home. Her blood cortisol level was measured in her last visit with suspicion of adrenal insufficiency, which was revealed to be low (less than 0.5 μg/dL). A high ACTH, low aldosterone, and normal renin were also reported, indicating secondary adrenal insufficiency. Furthermore, T4 and TSH tests were performed, reporting a TSH lower than 0.2 in the initial TSH test, which was revealed to be normal after the test was repeated. Subclinical hyperthyroidism was indicated, given the high TRAb figure. The patient underwent treatment with fludrocortisone and prednisolone, which improved her condition. This report accentuates that diabetic patients with reduced insulin requirements must be checked for adrenal insufficiency immediately to prevent potential complications.

Abstract: 

Introduction: Problems associated with the field of Anesthesiology include stress related causes pertaining to the characteristics of the profession, concerns about the education process, insufficient skill attained and lack of protection against professional liability. This study is aimed at construction of a testing instrument to quantify stresses specified to the anesthesiology residents. Based on theoretical discussions as and literature published well as about the sources of stress in this specially and other professions, we compiled a primary checklist of stressors. Then, we asked specialists, professors and residents to verify face validity. Using their comments, we revised the checklist and used it in this study. Next we evaluated the content validity and reliability of the test and used loop method to qualify the effect of each question on the reliability of the test. We performed factor analysis (exploratory method) to evaluate construction, validity and used preliminary form of confirmatory method to validate correctness of five assumed factors. Using factor analysis, five previously assumed factors were reduced to three principal factors and a number of additional (miscellaneous) questions. Obtained results show that the questionnaire is reliable and valid in face and content as well as construction. The results of a complementary study using this questionnaire among another group of residents in the end of year 2003, confirms the ability of questionnaire in measuring the above mentioned factors.

Abstract: 

Background: Vitiligo is an acquired multifactorial disorder. Different treatments are used for it and all of them have adverse side-effects. Vitiligo is an autoimmune disease associated with other autoimmune diseases such as hypothyroidism, hyperthyroidism, rheumatoid arthritis, diabetes mellitus, and so on. In these diseases, deficiency of vitamin D and B12 are proposed. So in this study, treatment response and remission in patients taking oral vitamin D and B12 was administered.Methods: 60 patients with active vitiligo were randomly divided into three groups. Group I received topical PUVA for 6 months; group II received topical PUVA and 1 cc oral drops of vitamin D3 daily for 2 months, and then topical PUVA alone for following 4 months; and group III received topical PUVA and 1 cc oral drops of vitamin D3 daily and sublingual tablets of vitamin B12 daily for 2 months, and then treated with topical PUVA for following 4months. Cure rate and side effects were evaluated using Vitiligo Area Scoring Index (VASI) at the third and sixth months. Findings: In each of the three treated groups at different times, the average changes in the area and extent of the lesion were not significantly different. Pruritus and erythema occurred in some patients.Conclusion: In this study, the reduction in the extent and area of lesions in the group receiving vitamin D compared to the control group were higher that indicated the role of vitamin D in preventing progression of active vitiligo.

Abstract: 

The aim of this study was to examine the diagnostic accuracy of the Addiction Acknowledgment Scale (AAS), Addiction Potential Scale (APS), and MacAndrew Alcoholism Scale-Revised (MAC-R), and to determine optimal cutoff scores for these scales.140 students from university of Tehran and 81 addicted persons seeking rehabilitation (matched for age) were selected for this study.Since these scales were derived from the MMPI-2, which is used to diagnose psychological disorders, a pilot study was conducted to examine their diagnostic accuracy in order to identify psychiatric patients (n=78) and addicted persons (n=81). The AAS had good diagnostic accuracy and the MAC-R was relatively accurate. After the pilot study, we examined the diagnostic accuracy of these Scales. The AAS was the only scale having internal consistency at a satisfactory level. The mean differences of the 3 scales were statically meaningful for the two groups (students and addicted persons). Results of the Receiver Operating Characteristics (ROC) analysis and sensitivity analysis showed that the AAS was the most accurate among the scales and appeared to offer considerable promise as a substance abuse screening tool. In contrast, the APS had the lowest diagnostic accuracy among the three scales, yielding results that would be of minimal clinical use.
